How do you get a full medical record, including doctors' and nurse practitioners' comments from a hospital in the USA?
I requested and received my record from the hospital but it didn't include the notes, just a list of things I was treated for.

you would have to contact medical records. They may not copy all the notes. You will also have to request in writing and pay a copy fee.
