In London if you’re getting a hotel room for 2 people do both of you need to show ID at reception or just the person who is paying?

Just the person who is paying. They will ask for a credit card and ID for the person the room is book under. However, more and more hotels are trained to recognize human trafficking situations and they may raise some alarm if the other person obviously looks underage.

Usually it's both people, but some hotels just ask for the ID of the person who is paying.

Depends. Some hotels require ID for all guests, most just for the person who's booked / is paying.
