Of course it has happened. More often than not the tank was just maneuvering and didn't actually care about the soldier that was run over.

Yes. During the chaos of battle, soldiers have been run over by tanks and APC's. There have been veterans interviewed for Vietnam war documentaries stating that when their perimiter was breached by the enemy at night, the M113 APC's were driving over US soldiers durign the chaos because the drivers couldn't see the soldiers in their foxholes. They saw their friends ripped to pieces under the tracks.

Sure. In my early days in the military, a soldier went to sleep under a tank on a very rainy day. Over time, the tank sand into the mud while he slept, crushing him.

No. Never. Every soldier who has ever found himself in front of a tank has just been lucky they went around.
