You didn't specify a cruise line. Anyway, go on the cruise line's website and look at the FAQ. This is always on there. Generally, if it is a closed loop cruise (leaving and returning to the same US port), all you need is a birth certificate. That said, it's a good idea for kids to have passports. I got them for my kids the first time they went on a cruise when they were 5 and 7 years old. It just makes things easier.

You don't need one until you are 16. Bring the child's birth certificate.

Why not ask the cruise line. They will have the definitive answer for you

I generally recommend cruising with a passport but its possible to do so without one If the child is a US citizen going on a closed loop cruise with his or her parents, the minimum that is needed is a certified copy of their Birth Certificate. Carnival Cruise Lines even allows for a clean photocopy of their BC. However, you have to understand the ramifications of cruising without a passport......

Contact the cruise line for their current rules.

Yes. You apply at your local post office. They usually require both parents to be present, but they will accept a letter stamped by a notary public from one parents saying they can't be present, but the other parent has permission to travel with the child. Children's passports have to be renewed every 5 years (because their faces change so much).

Depends the citizenship of child and destination of cruise. Generally, all U.S. citizens are required to present a valid U.S. passport when traveling to The Bahamas, as well as proof of anticipated departure from The Bahamas. Those traveling to The Bahamas on a cruise may use another Western Hemisphere Travel Initiative (WHTI) compliant document. However, we strongly recommend visitors obtain a passport before travel in case of an unforeseen emergency that requires a cruise passenger to disembark and return by air. U.S. travelers coming for tourism will not need a visa for travel up to 90 days. All other travelers will need a visa and/or work permit. Bad persons are after children. A child with one parent only gets extra attention. https://help.cbp.gov/app/answers/detail/a_id/827/kw/divert%20child%20abduction
