If the car has a lien on it- yes. But only comprehensive. If the car is paid for and in a garage most local authorities don't worry about it. If it is inoperable in some places that could cause problems. Keep in mind any damage done to the car sitting in the garage will not be covered. But with comprehensive insurance it might be. To give an example if the car is not insured and the garage catches fire it might be a total loss. If the car has comprehensive insurance and the garage burns then some compensation might be expected.

Depends on your local laws and if having valid insurance is a condition of registration, but assuming not, if the car is not in use then you don't need to insure it. Just make sure it is covered under your home policy if it has any value.

Depends on your local laws. In some locations it is subject to hulk laws and impounded if not licensed, you may not be able to license without insurance.
